Often during the school year, we take photographs and videotape of your children for use in the classroom.
We also will be using photographs of students on our field trips, to be included in class projects and books and on our classroom web page. Please indicate below if you give
permission for your child to be photographed or videotaped for classroom purposes. We will also be creating a list of addresses and phone numbers of the students in our class. This list can
be used for social events outside of school and birthday party invitations. (Birthday invitations are not permitted in our classroom.) This is strictly voluntary. If you wish your child to be
included on this list, please download the form below and submit to the school office.
